Life! is funded by the Victorian government and managed by Diabetes Victoria. The program includes 7 sessions delivered over a 12-month period.

The Life! program offers a range of options:

  • Group course (face-to-face or online groups) – The Life! group course comprises 7 sessions and is delivered in-person or online via Zoom or Teams. Each group consists of 8-10 participants and is delivered by an accredited health professional. Online groups run for 45 minutes, and face-to-face groups run for 1.5-2 hours.
  • Telephone coaching service (individual calls) – The Life! Telephone Health Coaching (THC) option includes 7 phone calls with an accredited health professional. This delivery mode provides flexibility for participants to complete the program at a time and location that is convenient for them.
  • Life! program for Culturally and Linguistically Diverse (CALD) communities – Bi-lingual facilitators have been trained to deliver the program in Chinese, Vietnamese and Arabic. All program resources have been translated and adapted to be culturally appropriate for members of these communities. We also offer the program in plain English.
  • Road to Good Health program – The Road to Good Health program has been developed to support Aboriginal healthcare workers and health professionals working with Aboriginal communities, to encourage individuals, families and community groups to make healthy lifestyle choices. It offers a supportive, flexible and culturally safe environment for Aboriginal and Torres Strait Islander people to learn how to improve their diet, physical activity and stress management.

Please note, The Life! program is not available for people who are living with diabetes. The Life! program is a prevention program, offered to people who are at risk of developing type 2 diabetes, heart disease and stroke. The program equips people with lifestyle modifications and support to reduce their risk.

    After completing our 3 minute health check, choose the option that suits you:

    • Group course

      Life! group courses are available in most metropolitan and regional communities.
    • Telephone Health Coaching

      During Life! Telephone Health Coaching (THC) you will receive seven phone calls over 12 months.

    Introduction session

    One-on-one with your facilitator

    In this one-on-one session you’ll discuss your lifestyle and understanding your risk factors for type 2 diabetes, heart disease and stroke and work with your facilitator to set your health goals.

    Session 1 - Program overview

    Each session runs for: 1.5-2 hours face-to-face or 45 minutes for online groups

    Learn more about type 2 diabetes, heart disease and stroke, changing your lifestyle to improve your health and reduce your risk of chronic disease and understand energy balance and portion control.

    Session 2 - Healthy eating

    Held 2 weeks after session 1

    Learn more about nutrition – including reading food labels and modifying recipes – and set achievable healthy eating goals.

    Session 3 - Physical activity

    Held 2 weeks after session 2

    Why physical activity is important, understand how to incorporate more exercise into your lifestyle and set achievable physical activity goals.

    Session 4 - Wellbeing

    Held 2 weeks after session 3

    Learn more about the importance of stress management, relaxation and sleep, Eating out tips, How to manage setbacks and being supported to reach your goals.

    Session 5 - Progress

    Held 4 months after session 4

    Understand more of how to manage setbacks and barriers and, review your progress and goals, and setting any new long-term goals.

    12-month session - How are you going?

    Discuss any challenges you have faced, review your goals and set new ones. Work on managing setbacks and tackle barriers and challenges.

    Am I Eligible

    You can check whether you are eligible for the Life! program by doing our simple online test. People who are living with diabetes are not eligible for the program.

    Step 2

    Did you score less than 12 on the online test? You may be still eligible for the Life! program if you are:

    18 years or over and have been diagnosed with one or more of the following;

    • heart disease or stroke (e.g. heart attack, arrythmia, angina, heart failure)
    • diabetes during pregnancy
    • chronic kidney disease
    • high cholesterol
    • high blood pressure
    • high blood glucose levels
    • polycystic ovarian syndrome

    45 years or over, or ≥ 30 years and of Aboriginal and/or Torres Strait Islander descent and have an Absolute Risk score of ≥ 10% when referred by a GP clinic.
